Ontario Health posted a survey on their site. Click here to sign up and get started! The first 33,000 people will receive a $10 gift card (choose out of numerous retailers) for completing the survey! The survey takes about half hour. I got an email confirmation after completing the survey asking me to pick which retailer I would like the gift card for.
